I understand that participation injudo and realted training activities at Barrie Judo Club involves inherent risks, including physical contact, throws, falls and potential injury.

By checking this box and signing electronically, I voluntarily assume all risks associated with participation and release Barrie Judo Club, it's directors, instructors, coaches, volunteers, and affiliates from liability for any injury, loss or damage arising from participation, to the fullest extent permitted by the laws of Ontario.

I confirm that I (or my child) am physically fit to participate and will inform the club of any medical conditions or injuries that may affect safe participation.

I acknowledge that I have read, understand and agree to comply with Barrie Judo Club's policies, including but not limited to:

I understand that failure to foloow club policies may result in suspension or terminiaton of membership without refund.
